健康证认证所需资料：

1、健康证公证书

注：1、需在户口所在地公证处（具有涉外资质）做成公证书。       

     2、对公证书证词及翻译，不同大使馆要求不同，请参考

     3、公证书时间长不超过半年，不同大使馆要求不同
